Get a New Passport, Applicant Must Meet the New Passport Qualifications:
- Applicant is a Us citizen over 18 years old. AND:
- Applicant has never had a U.S. Passport issued before. OR:
- Applicant was under 16 years old when most recent passport was issued OR:
- Applicant has not had a U.S. Passport issued for over 15 years.
Original Passport
valid for 6 months beyond stay, and with at least one blank visa page available for visa stamp.
NOTE: Passport must have at least one blank visa page and must not expire within the next six months. If you require passport services;

Online Visa Application

In addition to the online application page, you must also print out each page that you complete before moving on to the next. This is temporarly being requested by the consulate due to technical issues with their online systems.

2 x 2 Passport Photo
One Passport Photograph, must be 2x2 inches in size and taken within the past 6 months. ** If you can, glue your photo to the photo section of your visa application
Proof of Residence (Jurisdiction)
Please provide a copy of your Driver's License. If you do not have a Driver's License/State I.D., you may send a recent Utility Bill or Voters Registrar's Card.
