To Get a Passport Pages, Applicant Must Meet the Following Qualifications:

have used all or most of the available visa pages inside a valid passport OR;
whose passport will not expire within the next year OR;
whose passport is not damaged or mutilated OR;

Original Passport

Valid/signed U.S. passport. .

1.2

Completed DSP-4085 Form

This form must be completed online and printed on two pages (not back to back). The passport office will not accept hand-written applications. This form must have the original signature of the applicant.

Two Signed Letters of Authorization. (PRINT TWO)

Please select the authorization letter that matches the processing location and print two copies. Both must have original signatures..

Proof of Travel

Applicant must submit a computer-generated itinerary from a travel agency or online booking agency showing departure from the U.S. in order to get new passport quickly.

If traveling on business and no itinerary is availabel, a letter on company letterhead indicating dates and purpose of travel will suffice for new US passport applications. If applicant does not posess proper itinerary. Please contact us for options.

Government Fees (Must be by Applicants check or money order only)

A check or money order made payable to "U.S. Department of State" for $142. This fee may also be paid online via credit card when filling out our online order form below.

Please include your date of birth on the memo line of the check. Starter checks for new bank accounts are NOT acceptable.

If you have had Name Change

If you have legally changed your name and you wish to have your new Passport issued in your new name, you must submit an original or certified copy of your marriage certificate, divorce decree, or court order showing the name change. Your new Passport will be issued in your new name.

Additional Info (skip if you do not apply)

If you have a temporary Passport, issued at a US Embassy overseas or other situation causing a short term Passport issuance, you must include a certified birth certificate, past US Passport, or Naturalization Certificate.

If you are behind in child support payments, you may not be able to get a passport.
